Party Request Form
Booking with us is simple and easy. Just fill in the booking form below and a team member will contact you via email or phone regarding you Spa request. Please don't forget you have to submit the payment for your reservation in 48 hours after approval.
A $100, $50 or $25 non-refundable deposit is required to secure your reservation. We accept Visa or MC via Pay pal.
There is a $50.00 non sufficient fee in the event that your cheque is not honored by the bank.
The Final balance is due before receiving services on the day of your event. Payments are to be made in Credit card or Cash only. Payments should be remitted upon our arrival to set-up your party.
Please Note: By continuing with the booking process, you are agreeing that you have read our store policies and party regulations. Once a payment is made, this signifies that you are agreeing to abide by all of Glam Dolls Salon & Spa Policies. (Please see policy page)
